ED STEVENS VOLUNTEER AWARD <br />Nomination Award <br />PURPOSE <br />The Sixth Annual Ed Stevens Volunteer Award will be presented by the North <br />Suburban Senior Council, Inc. and Community Education, a department of the <br />Roseville Area School District 623. The award is given in honor of Ed Stevens, <br />founder and first President of the North Suburban Senior Council, Inc., to an older <br />adult who is making outstanding contributions to his or her community through <br />volunteerism. <br />This award is in memory of Edwin Stevens, Sr., whose vision, dedication and <br />perseverance was instrumental in the establishment and success of the North <br />Suburban Senior Council, Inc. and the Roseville Area Senior Program. Ed was <br />among the first group of residents who worked to bring services for older adults into <br />the Roseville area community, including Meals on Wheels. <br />Please supply information about your nominee on the attached form, telling us <br />why you think she/he should receive this prestigious award. The nominee must be at <br />least age 60 and involved in volunteer work in the geographic area served by <br />Independent School District 623, consisting of all or parts of Arden Hills, Falcon <br />Heights, Lauderdale, Little Canada, Maplewood, Roseville and Shoreview. <br />Nominations must be submitted by organizations. Each organization may submit one <br />nomination. <br />Judging based on: <br />• Focus on volunteer contributions made to the community after age 60 <br />Variety of volunteer activities in which nominee is volunteering. <br />(such as volunteering with senior citizens, religious, <br />political, community, youth work) <br />Judges will look for accomplishments and scope of activities over the years, the <br />amount and quality of service performed after the age 60, quality of activities, and the <br />variety of services performed. <br />Use only this nomination form and fill it out completely. BE EXPLICIT and list only <br />volunteer work for which there was no Compensation.
