Draft <br />Dear Youth Athletic Association Applicant: <br />Thank you for your interest in Falcon Heights Parks & Recreation athletic fields <br />facilities. The Following is the youth athletic association application procedure for the <br />year 2006. <br />Enclosed you will find the following documents: <br />- Falcon Heights Parks & Recreation Youth Athletic Association Application Form <br />- Guidelines Establishing Priority for the use of City Facilities. <br />Associations must complete and/or provide the following items in order to apply; <br />- Falcon Heights Parks & Recreation Youth Athletic Association Form. <br />(Applications must be filled out completely in order to be considered) <br />- A certificate of insurance naming the City of Falcon Heights as co-insured. <br />Associations wishing to use Falcon Heights facilities during 2006 year must submit <br />applications no later than February 24th. Submit application and certificate of insurance <br />to: <br />. Falcon Heights Parks & Recreation <br />Attn: Lisa Anderson, Recreation Supervisor <br />2077 W. Larpenteur <br />Falcon Heights, MN 55113 <br />Applications will be reviewed and decided by City staff on March 10, 2006 <br />Once the application procedure is complete, association will be assigned priority for <br />facility use based on the guidelines listed in the Policy Establishing Priority for use of the <br />City Athletic Facilities document.
