• Page 6 <br />3) The name of the general manager, corporate officers, <br />proprietor and other person in charge of the premises to be <br />licensed, and all the information about said persons as is <br />required in paragraph B of this section of this code. <br />4) A list of all persons who own or control an interest in the <br />corporation or organization or who are officers of said <br />corporation or organization, together with their addresses <br />and all the information regarding such persons as is <br />required in paragraph B of this section of the code. <br />7. Application and Investigation Fees <br />The fees for a therapeutic massage enterprise licenses shall be as set <br />forth in section 4.01 A of this code. An investigation fee shall be <br />charged for therapeutic massage enterprise licenses as set forth in <br />section 4.01 A of this code. Each application for a license shall be <br />accompanied by payment in full of the required license and investigation <br />fee, if applicable. <br />• <br />8. Application Verification and Consideration <br />The city shall verify the information supplied on the license application <br />and shall investigate the background, including the criminal background, <br />of the applicant to assure compliance with this section. Within ninety <br />(90) days of receipt of a complete application and fee for a therapeutic <br />massage enterprise license, the city shall make a written <br />recommendation to the city council as to issuance or nonissuance of the <br />license. The city council may order and conduct such additional <br />investigation as it deems necessary, but shall grant or deny the <br />application within one hundred twenty 1120) days of receipt by the city <br />of the complete application and fee. At an advertised public <br />hearing the city council shall approve or deny the therapeutic massage <br />enterprise license. <br />9. Persons Ineligible for License <br />No therapeutic massage enterprise license shall be issued to a person <br />who: <br />• <br />
