PUD Application <br />2870 Middle Street. <br />Description and Reason for Request: <br />Amendment to existing PUD for owner occupied office development to allow purchase <br />and occupancy by social service organization providing life care services to women <br />including counseling and other services. <br />Our life care center is one of 22 metro-wide centers. <br />Our center has been providing services since 1982. <br />We provide lay counseling. <br />Self-administered pregnancy test. <br />Medial referrals <br />-material assistance <br />-adoption referrals <br />Financial applications to help with housing related to pregnant client. <br />Earn & Learn Program -promotes responsibility of clients. Done by appointment only. <br />Emergency assistance -material goods. <br />Expect to have minimal change to current hours: <br />Monday 5-8 p.m. <br />Tuesday 12-3 p.m. <br />Wednesday 12-3 p.m. <br />Thursday 12-3 p.m. 6-8 p.m. <br />Friday Closed <br />Saturday Closed <br />Sunday Closed <br />4-8 cars maximum at any given time, includes the 3 staff aC location. <br />Board of Directors meets one Wednesday night per month: <br />7-10 p.rn. (8 cars) <br />
