MEMORANDUM <br />TO: Hazardous Materials Evaluators <br />FROM: Joel Hanson, City Administrator <br />DATE: November 10, 2015 <br />RE: Request for Hazardous Material Survey Proposal <br />The City of Little Canada is seeking quotations for a Hazardous Materials Survey to be <br />conducted on four single family homes prior to demolition. The houses are all unoccupied. The <br />property addresses are as follows: <br />• 30 Little Canada Road, E. <br />• 56 Little Canada Road, E. <br />331 Little Canada Road, E. <br />• 2801 Country Drive <br />A Location Map of the properties to be evaluated is enclosed and photos of the properties are on <br />the following pages. <br />As soon as the survey is completed, the City will obtain quotes for abatement work. Once <br />abatement work is completed, we will be demolishing the structures. We are looking for a report <br />that ensures that any hazardous materials requiring abatement pursuant to applicable standards of <br />Ramsey County, the State of Minnesota and the U.S. Environmental Protection Agency are <br />identified. <br />The firm awarded this job must provide workers compensation insurance at the mandated <br />statutory limits. The contractor shall also provide comprehensive general liability coverage with <br />minimum limits of $1,000,000 per occurrence. The City of Little Canada shall be named as an <br />additional insured and be provided with a certificate of insurance with a minimum of ten (10) <br />days' notice of cancellation. <br />The property is available -for on-site inspection during normal working hours (upon request) by <br />contacting the City of Little Canada at (651) 766-4029. <br />The price submitted must utilize the attached form and encompass all costs for all <br />structures. Supplemental attachments are acceptable as long as clearly delineated. The <br />quotation must also indicate when the completed reports will be submitted to the City. <br />The award will be considered based on the lowest responsible quotation while also considering <br />the date of report completion. The owner reserves the right to reject any and all quotations and <br />to make the award in its best interests. <br />Quotations may be mailed, faxed, or e-mailed to the City of Little Canada. They are due by <br />12:00 p.m. on Thursday, November 19th They must be signed by an authorized <br />representative of the company and dated. <br />
