Paid Time Off (PTO) Donation <br />PURPOSE <br />This policy establishes the procedure through which eligible employees may voluntarily donate a <br />portion of their accrued Paid Time Off (PTO) time to be converted to assist another employee <br />who has exhausted all forms of paid leave due to a "medical emergency." <br />A medical emergency is defined as a physician's diagnosis of a major life threatening event of an <br />employee or immediate family member that will require the employee to be absent a prolonged <br />period from their employment duty and will result in substantial loss of income to the employee <br />because the employee will have exhausted all paid leave available apart from the leave donation <br />plan. Immediate family for the purpose of this policy shall include employee's spouse, children, <br />parents, grandparents, grandchildren and includes the children, parents, grandparents, and <br />grandchildren of the employee's spouse, and any person regularly residing in the employee's <br />immediate household. <br />A qualifying major life-threatening event includes but is not limited to heart attack, stroke, <br />cancer, organ transplant, life threatening illness or condition as defined by a physician's <br />diagnosis using the FMLA medical certification of the catastrophic health problem. <br />POLICY <br />With the written consent of the City Administrator, employees having accrued PTO time can <br />donate a portion of such accrued leave to fellow employees experiencing a qualifying event. The <br />City Administrator has the right to deny or limit use of donated leave if it is determined to be in <br />the best interest of the City. <br />ELIGIBILITY <br />Employees must be benefit -eligible in order to participate as a recipient or donor in the PTO <br />Donation Progam. Requests for participation in the PTO Donation Program will be accepted <br />only for individuals who are defined as having an approved medical emergency and who are <br />within one pay period of exhausting all accrued paid leave. Employees receiving Workers' <br />Compensation benefits from city -related injuries or illnesses are not eligible to receive donations. <br />CONDITIONS <br />The terms and conditions governing the PTO Donation Program are as follows: <br />Forms: Completed donation forms must be submitted to the City Administrator for approval. <br />The approval remains valid until the employee notifies the City Administrator to annul the <br />approval or the employee becomes ineligible to participate in the program. <br />4 <br />
