Contract No.: <br />Address/City/State/Zip: 14475 Quiram Drive, Rogers, MN 55374 <br />Total Supplemental Agreement Amount $ $151,837.50 <br />This contract is between the City of Little Canada and the Contractor as follows: <br /> <br />WHEREAS: This Contract includes, among other things, the reconstruction of <br />Centerville Road from station 504+90 to 550+00; and <br /> <br />WHEREAS: Clayey loam soils significantly above optimum moisture were encountered <br />during the pipe excavation between approximately station 523+00 to <br />537+00. <br /> <br />WHEREAS: The Engineer has determined that approximately 1400 feet of street <br />require geotextile fabric, 2 feet of select granular borrow, and drain tile at <br />the low point will be necessary in the areas of the wet clay material to <br />provide for an adequate street base and that this work sho uld be <br />completed as part of the project. <br /> <br />NOW, THEREFORE, IT IS HEREBY MUTUALLY AGREED AND UNDERSTOOD <br />THAT: <br /> <br />1. The Contractor agrees to complete the above described changes and additions <br />to the project. <br /> <br />2. This Agreement does not change any of the contract provisions unless otherwise <br />identified in this Agreement. <br /> <br />3. Payment for the work was reviewed and approved by the Engineer. <br /> <br />4. The Contractor will not make claim of any kind or character for any other costs or <br />expenses that he may have incurred or that he may hereafter incur in performing <br />the work or furnishing the materials of this agreement. <br /> <br /> <br />Estimate of Cost: <br /> See attached spreadsheet. <br /> <br /> <br /> <br /> <br /> <br /> <br />
