Contract No.: Address/City/State/Zip: 14475 Quiram Drive, Rogers, MN 55374 Total Supplemental Agreement Amount $ $28,182.72 This contract is between the City of Little Canada and the Contractor as follows: WHEREAS: This Contract includes, among other things, replacement of existing impacted bituminous trail and installation of new concrete sidewalk; and WHEREAS: The City determined that concrete sidewalk should be used to replace the existing bituminous trail that was proposed for replacement along Centerville Road; and WHEREAS: The City also felt that the remaining bituminous trail from Little Canada Road to the removal limits on the plan (approximately 240 feet) should be removed and replaced at this time and also be replaced with concrete sidewalk; and WHEREAS: The Engineer determined that the modifications should be completed with the existing project work. NOW, THEREFORE, IT IS HEREBY MUTUALLY AGREED AND UNDERSTOOD THAT: 1. The Contractor agrees to complete the above described changes and additions to the project. 2. This Agreement does not change any of the contract provisions unless otherwise identified in this Agreement. 3. Payment for the work was reviewed and approved by the Engineer. 4. The Contractor will not make claim of any kind or character for any other costs or expenses that he may have incurred or that he may hereafter incur in performing the work or furnishing the materials of this agreement.
