<br />SECTION 13 – OTHER INSURANCE BENEFITS - SHORT TERM DISABILITY (STD) <br />Purpose <br />The City provides Short Term Disability (STD) coverage at no cost to all regular full-time and <br />part-time employees who accrue benefits as a supplement to the PTO program. The intent of <br />STD is to bridge the gap between banked PTO or deferred sick time, and the 90th day when an <br />employee may qualify for Long Term Disability. No enrollment forms need to be completed. <br />Participation is mandatory and Employees are automatically enrolled once they are eligible. This <br />benefit is provided through a self-funded City account and there is a cost to full-time employees <br />of $10 per month (post-tax dollars), with that cost pro-rated based upon an employee’s percentage <br />of a 40-hour work week for eligible part-time employees. Should the use of STD deplete the <br />accrued balance, the City will contribute funds to bring the balance back to $0.00. <br />If, due to a disability, an employee is absent from work for more than 14 consecutive calendar <br />days, the City will continue the employee’s salary at up to 2/3rds of the base pay amount from the <br />15th calendar day through up to the 89th calendar day of absence. <br />The employee must provide documentation from a physician certifying that the employee cannot <br />return to work in some capacity the City finds acceptable. The City Administrator is responsible <br />for administration and the program guidelines for STD. The City reserves the right to modify or <br />terminate the program at its discretion. <br />Eligibility <br />STD is available only for an employee’s non-work related disability. It may not be used to care <br />for ill or injured family members. The employee or their representative must submit all <br />paperwork stipulated by the City Plan Administrator in order to qualify for benefits. If an <br />employee’s injury or illness is determined to be covered under workers’ compensation, it is not <br />eligible to be covered under this STD program. <br />If an employee’s disability restricts the number of hours the employee may work, they are still <br />eligible for compensation. The STD coverage will bridge the difference between the hours the <br />employee is able to work and the 2/3rds cap. <br />Definition of a Disability <br />A disability is defined as a physician-certified, non-work related, accidental injury or illness <br />(physical or mental) including pregnancy that precludes the employee from performing the duties <br />of their job description in some capacity that the City finds acceptable. A determination that an <br />employee is able to work a modified schedule (i.e., part-time) would not disqualify them from <br />eligibility to receive benefits. <br />
