<br />Budgeted revenue for PHS, its consolidated affiliates, and its managed/partnered facilities, for the fiscal
<br />year ending September 30, 2020 is approximately $496,000,000.

<br />The mission of PHS is to honor God by enriching the lives and touching the hearts of older adults and its
<br />vision is to provide more choices and opportunities for more older adults to live well.

<br />History
<br />PHS is a Minnesota nonprofit corporation formed on October 27, 1950. PHS is exempt from federal
<br />income tax under Section 501(c)(3) of the Code. PHS, in its capacity as Limited Guarantor, will provide a limited
<br />guaranty of repayment of the Series 2020 Bonds pursuant to a Limited Guaranty Agreement, dated as of May 1,
<br />2020 (the “Limited Guaranty”), the proposed form of which is included in “APPENDIX D – DEFINITIONS AND
