Application for Tax Exempt Issuance Authority; <br />2. Sworn Applicant Statement with Sworn Certification of Borrower and Sworn <br />Certification of Certified Public Accountant; <br />3. Preliminary Resolution; <br />4. Statement of Bond Counsel; and <br />5.
