PID results will be reported to the client within 12 hours. Collect (4) samples for rus/e chemical analysis by a laboratory using EPA approved testing protocols for the presence of Diesel Range Organics (DRO),Gasoline Range Organics (GRO/BETX/MTBE) and RCRA Heavy Metals or as predicated by the fmdings of the aforementioned ESA. Issue a report of the findings. Option B Geotechnical Study $4,1.50.00 Clear Utilities with Gopher State One Call for (10) standard penetration soil borings 20' below grade. Our Drill Truck is considered Aheavy construction equipment" therefore, we are not responsible for damages to yard, trees or any other structures. Engineering requirements specify that drilling must continue 15' past any fill (disturbed soil). Shelby tube samples and unconfined compression laboratory analysis an extra $250.00 each and dry density and moisture tests an extra $75.00 each, if required by Geotechnical Engineer. Additional soil borings $10/Ft from 0 to 25' deep, $12/Ft fiorn 25' to 35' deep, $14/Ft from 35' to 40' deep, etc, plus $150.00 for laboratory analysis and geotechnical report. Consistent with Mn Dept of Health (MDH) Rule 4725, "regulated borings" into aquifers or borings >25' or beyond a confining layer may require additional borehole/well sealing provisions at $125/boring. American Society of Testing Material (ASTM) requires drilling 15' into good soils in order to write geotechnical report. TERMS AND CONDITIONS The client agrees to provide us with all past environmental reports, studies and correspondences with particular attention afforded to the previous onsite UST closure event. The client will be invoiced on a monthly basis with payment due upon receipt. Visa & MasterCard payments must be posted within 10 days of receipt of our invoice and may be subject to a 5% surcharge. Notwithstanding separate agreements between parties, a finance charge of 8% APR will be assessed against any unpaid or past due balances. MTM Environmental, Inc. reserves the right to collect damages for legal costs, collection fees or other costs resulting from client's failure to satisfy the terms contained in this covenant or institute a mechanics lien against the property consistent with State Statutes. The client will hold harmless and indemnify MTM Environmental against claims resulting from loss, damage deficiency or liabilities resulting from errors and omission of facts. MTM Environmental, Inc.
