City of Little Canada <br />AET Proposal No. 3 -07 -170 <br />May 4, 2007 <br />Page 2 of 3 <br />Scope of Services <br />In order to achieve the purpose of this service, AET will perform the following tasks: <br />• Observe the buildings to obtain information indicating the existence of ACM and <br />hazardous materials. <br />• Locate and sample, where accessible, thermal system insulation, surfacing material, and <br />miscellaneous material identified as suspect ACM. <br />• Analyze the suspect ACM samples for asbestos content. <br />• Assess the condition and estimate the quantities of any confirmed ACM. <br />• Inventory identifiable items that potentially contain hazardous materials such as mercury, <br />poly - chlorinated biphenyls (PCBs), chlorofluorocarbons (CFCs) or other materials. <br />These items may include but are not limited to thermostats, switches, fluorescent lights, <br />batteries, transformers, light ballasts, refrigeration units, chillers and door closures. <br />• Preparation of a written report. <br />Limitations <br />The areas we select to be sampled and inventoried will be based on visual observations of the <br />above- identified building. Material from areas that are concealed or inaccessible, including <br />inaccessible for safety concerns, may not be discovered. <br />Client Responsibilities <br />The Client shall also provide AET with safe access to the buildings. <br />Performance Schedule <br />We will complete this project and deliver the report to you within two weeks of receiving <br />authorization to proceed. Please let us know if this timetable does not meet your schedule as we <br />are available to consider any special needs that you may have. <br />This proposal is valid for a period of 90 days from the date issued. <br />Conditions <br />The attached Service Agreement applies to this project. <br />Fees <br />Our fees for services for the scope of services described above will be a lump sum, fixed fee of <br />$2,600.00. This fee includes laboratory analysis of up to 75 samples for asbestos. Additional <br />asbestos samples, if needed, will be sampled and analyzed for an additional cost of $25.00 per <br />sample. <br />
