<br /> 2202-3 <br />same fail to comply with, or have a history of violations of the laws or ordinances which apply to <br />health, safety, or moral turpitude. <br /> <br />6. The City Council may attach such reasonable conditions to the license as it, in its sole <br />discretion, deems to be appropriate. <br /> <br />7. The City may run criminal history checks on any applicant for a license pursuant to <br />this chapter. <br /> <br /> 8. A business license for massage therapy or complementary and alternative health care <br />practices conducted as an accessory use shall be reviewed annually. No license approved by the City <br />grants the licensee a property right or entitlement to a license. The City Council may refuse to issue, <br />refuse to renew, or may suspend or revoke a license for any reason deemed to be in the best interest <br />of the public health, safety, or welfare. The City shall not be liable for any damages, including but <br />not limited to direct, consequential, or incidental damages, deprivation of property, loss of income, <br />loss of profits, or loss of livelihood.A license for massage therapy business license as an accessory <br />use shall be reviewed annually. No license approved by the City grants the licensee a property right <br />or entitlement to a license. The City Council may refuse to issue, refuse to renew, or may suspend or <br />revoke a license for any reason, and the City will not incur liability for any damages including, but <br />not limited to, direct, consequential, or incidental damages, deprivation of property, loss of income, <br />loss of profits, or loss of livelihood. <br /> <br /> 9. Applications for individual <br />practitioner licenses under an approved business license shall be reviewed and acted upon by the <br />City Administrator, subject to compliance with all applicable requirements of this Code.Applications <br />for Massage Therapy Business Licenses will be reviewed and acted upon by the City Council. <br />Applications for Massage Therapist Licenses will be reviewed and acted upon by the City <br />Administrator, subject to compliance with all Code requirements. <br /> <br />2202.040. MASSAGE THERAPY AND COMPLEMENTARY AND ALTERNATIVE <br />HEALTH CARE PRACTICES ONLY BY LICENSED PERSONS. <br /> <br />1. No person shall provide massage therapy or complementary and alternative health <br />care practices without being licensed individually as provided in this chapter. <br /> <br />2. Application for license. Any person desiring to be licensed shall file an application <br />on forms provided by the City Administrator. The application shall contain such information as the <br />City Administrator may require, including: <br /> <br />a. The applicant's full name, address, social security number, and written proof <br />of age. <br /> <br />b. The name and address of the establishment by which the applicant expects to <br />be employed.
