• Sufficient information to understand the facts that led the complainant to believe that <br />discrimination occurred in a program or activity that receives Federal financial <br />assistance; and <br />• The date(s) of the alleged discriminatory act(s) and whether the alleged discrimination is <br />on-going. <br /> <br />Complainants are encouraged to submit complaints directly to Minnesota Department of <br />Transportation (MnDOT) via its online complaint form available here: <br />https://www.dot.state.mn.us/civilrights/nondiscrimination-complaint-form.html. <br /> <br />Complaints can also be filed by completing and submitting City of Little Canada’s Title VI <br />Complaint Form available at Appendix B or by sending an email or letter with the necessary <br />information to: <br /> <br />City of Little Canada <br />515 Little Canada Road <br />Little Canada, MN 55117 <br />Email: laura.linehan@littlecanadamn.org <br />Phone: 651-766-4029 <br /> <br />The complaint form is also available in hard copy at City of Little Canada City Hall at 515 Little <br />Canada Road, Little Canada MN 55117. Language assistance is available for limited English <br />proficient individuals. MnDOT has hard copy complaint forms available in Spanish, Somali, and <br />Hmong. Other languages can be accommodated as needed. <br /> <br />If necessary, the complainant may the phone number above and provide the allegations by <br />telephone.
