Page 2 <br />Distribution of LPA is based on yearly certifications for each qualifying city. Cities are <br />required to submit an annual certification in order to receive LPA payable in calendar <br />year 1999 and subsequent years. Enclosed is a copy of Form LPA -CI. This <br />certification must be submitted to our office by June 30, 1998 and both questions 1 <br />and 2 on the form must be answered "Yes" in order for your city to qualify to <br />receive LPA in 1999. If the Form LPA -CI is not received by the June 30, 1998 <br />deadline, or if either question 1 or 2 is not answered "Yes" or either qpestion is left <br />blank, your city will not qualify for LPA in calendar year 1999. <br />For qualifying cities, calendar year 1999 LPA will be paid in two equal installments on <br />July 20 and December 26 of 1999.
