Enclosed please find one (1) copy of the Cooperative Purchasing Agreement, which has been executed by the State of Minnesota and contains your CPV order permit number. Any previous agreement is now void. Please note that the agreement now provides for a renewal option for a period of up to five (5) years. This change was made for the convenience of the members in that it eliminates the need for organizations to reapply every year. Organizations can now renew its memberships simply by providing the annual administrative fee. PLEASE HAVE THE AGREEMENT SIGNED BY AN AUTHORIZED PERSON. MAKE AND RETAIN A COPY FOR YOUR RECORDS AND RETURN THE ORIGINAL TO SHERRY BROWN. For those members who have a FAX machine, your CPV order permit number /fax security access code will enable you to utilize the Voice /FAX system to receive contract releases. Please follow the instructions in the enclosed brochure titled, `Voice Fax System Access Instructions ". In the event you do not have a facsimile (FAX) machine, a Contract Commodity Index has been enclosed to assist you. It lists contracts now in force from which you may make purchases. Please submit, in writing, your requests for the actual contract release which contains all pertinent ordering information. When placing orders with the contract vendor, please state your current permit number and the applicable State of Minnesota contract number on all orders.
