Data <br />collection <br />forms <br />Rating System <br />Each local road agency will likely develop its own forms for collecting <br />condition information. There is a sample form on page 37 to help you <br />develop your own form. <br />Use one page for each roadway segment being evaluated, and leave <br />ample room for comments. Information on the history and construc- <br />tion of the roadway is very important. Drainage condition should be <br />rated and problems noted. You should also note unusual conditions <br />and isolated problems, such as poor soils or faulty construction. Be <br />sure to note the date the data was collected and include the names of <br />those doing the rating. <br />For evaluation purposes, divide the local road system into individual <br />segments which are similar in construction and condition. Segments <br />may vary between one -half mile to one mile long for rural sections, <br />one block to four blocks or more for urban areas. <br />You can also collect other information on pavement widths, <br />alignment, safety hazards, etc. <br />Page 53 <br />
