Little Canada Road, Ste. 1 <br />Dear Sir or Madam: <br />Grace Adult Day Center, Inc. ( "Center ") is hereby requesting approval of a Conditional <br />Use Permit to establish an adult day service center at the property located at 80 E. Little <br />Canada Road, Ste. 1. The facility is a 6600sf. facility and consists of two enclosed <br />offices and a warehouse space (see floor plan attached). <br />The owners of the business are: Macy Yang, Mai Kao Yang and Champa Hlub. <br />The owners are the primary operators and managers of the business. Staff will be hired <br />at the ratio of 1 to 5 senior participants as required by the state licensing department. <br />The primary contact for this application is Macy Yang, telephone number 651 - 239 -4068, <br />email MacyC@,1ChoiceConsultinq.com. <br />Business Description <br />An adult day center is a facility for community -based programs for seniors designed to <br />meet the health, social, recreational and therapeutic needs of elderly and disabled <br />adults. Adult day centers also provide much needed respite and peace of mind for <br />families and caregivers who need time outside of their care for the elderly and disabled. <br />Seniors attend the Center during the day and return home in the afternoon. <br />The Center will host daily activities such as table or board games and limited exercise <br />activities. There will also be television and other entertainment throughout the day. <br />Lunch and snacks will be provided by the Center to the participants on a daily basis. <br />Catered lunch will be brought into the Center from outside and light snacks will be <br />prepared on -site. <br />Seniors who want to participate in the program must apply to the County. The County <br />conducts a personal interview to determine the applicant's eligibility based on his or her <br />health, independence and other eligibility for this program. The County social worker <br />than approves the number of hours or days that the senior participant may engage the <br />Center. Once approved, the County enters into a contract or agreement with the Center <br />to reimburse the Center for costs. <br />
