NORTH SUBURBAN YOUTH ASSOCIATION <br />GRANT APPLICATION INFORMATION <br />The policy of the NSYA is to accept grant application requests, for consideration, from organizations <br />and /or individuals within NSYA boundary areas. Grants are approved specifically for the purpose of <br />capitol improvements, hardships, and special projects for youth related activities. <br />The goal of NSYA, when providing funding, is to increase and maximize the number of youth served, <br />in a safe healthy environment, in our community. NSYA accepts fund request grant applications from <br />youth organizations and individuals that meet youth requirements. When individuals are requesting funds <br />to participate in an organized program, a representative of that organization must accompany the <br />individual to the interview. Preference in approving grants will be given to individuals participating in <br />local programs <br />NSYA suggests that, in addition to the NSYA, organizations and /or individuals making application, <br />submit the request to several other service organizations in an effort to lessen the commitment of NSYA. <br />If you have not applied to other sources for funding, be prepared to explain the reason at the interview. <br />The following information is required on the grant application: Name of Organization/Group /Individual <br />Making Application; Date of Application; Contact Name, Title, Address, Zip, Phone; Description of <br />Organization and /or Individual Background; Financial Information; Statement of Need; and Amount <br />Requested (including three (3) bid proposals, if applicable. <br />The application must be submitted thirty (30) days prior to the NSYA monthly membership meeting. <br />Membership meetings are scheduled on the last Tuesday of each month. All information requested on <br />the grant application must be provided to NSYA before a grant application will be considered. <br />Government, School Districts, and their affiliates may submit grant requests. If approved, a letter of <br />acknowledgment and acceptance must be received from the governing administration before funds can <br />be disbursed. <br />For action to be taken on a grant application, a representative from the organization, or the individual <br />making the request, is required to be present at the NSYA membership meeting for presentation and to <br />answer questions regarding the request. <br />For more information regarding the NSYA grant application process, contact Bob Matson (638- 4600). <br />Mail completed grant applications to: <br />North Suburban Youth Association <br />1751 West County Road B, Suite 107 <br />Roseville, MN 55113. <br />Page 37 <br />
