PAUL, MINNESOTA 55110 (612) 4260607 <br />Commercial Insurance Brokers <br />City of Little Canada <br />515 Little Canada Road <br />Little Canada, Minnesota 55117 <br />gg-2/VIQ WC7"71 <br />MAY 14 1986 <br />CITY OF <br />LITTLE CANADA <br />Subject: Loss Control Survey, City Hall , 515 Little Canada Road <br />Attention: Mr, Joseph Chlebeck <br />Dear Joe, <br />MAY <br />CTY <br />UTTLE C . <br />The insurance company surveyed your premises and operations and have come up <br />with some recommendations that will help in your loss control effort. <br />These recommendations are as follows: <br />LOCATION SURVEYED: City Hall, 515 Little Canada Road <br />1) The main control valve for the automatic sprinkler system should be <br />secured to prevent unauthorized shut off. This can be accomplished <br />with a padlock unit or electronic monitoring system. <br />GENERAL OPERATIONS: <br />1) It would be desirable for the city to begin documenting regular safe- <br />ty checks of the playground equipment. Depending upon extent of use - <br />age, periodic physical checks should be made of the playground equip- <br />ment for proper maintenance and upkeep 'of the equipment and written <br />documentation of these checks. <br />2) Because of the city's maintenance of ice rink facilities that exist <br />on school property, it would be highly desirable for the city to main- <br />tain a formal contract arrangement with the school in this regard. I <br />suggest that you contact the city's attorney for guidance in drafting <br />such a contract so that appropriate hold harmless arrangements can <br />be made in the city's favor. <br />3) It would be highly desirable for the city to require insurance cover- <br />age for units participating in parades sponsored by the city. The city <br />should also require being named as an additional insured and hold harm- <br />less arrangements with parade units. <br />LI ) <br />As a reminder, the City of Little Canada should be requiring certifi- <br />cates of insurance, additional insured endorsements, and hold harmless <br />arrangements for all individuals and /or businesses doing work for or on <br />behalf of the City of Little Canada, Review of these certificates <br />should be centralized in the city's operations to maintain standardi- <br />zation and accountability for getting these certificates. <br />3 <br />
