Dental Plan 1. Annual check -ups — NO CHARGE. This includes: examination and diagnosis, cavity (not detection x-rays, g , luo cancer exam, and oral (not scaling), hygiene instructions. Note: You save on preventive care up to $ 50 per person off usual charges. 2. Other fees at SPECIAL GROUP RATES. 3. The average family SAVES from 25% to 50% and MORE on dental bills. There are currently 12 conveniently located dental offices in the Twin Cities and suburbs, with more scheduled to open soon. Legal Plan This plan is in use by hundreds of different groups throughout the United States. Individuals in these groups must pay $50 per year for the plan. CAPS members get the plan at no extra charge as a CAPS membership service. phone 1. Legal questions answered by a simple p call to one of our participating attorneys — NO CHARGE. 2. Two half -hour visits to the law offices each year — NO CHARGE. 3. Unlimited legal office work other than listed above and as detailed in your legal certificate, at a maximum charge of $35 per hour. Family Counseling Plan This is a service that is directed toward the individual and his family In facing everyday problems. It is provided by psychiatrists, psychologists, registered nurses, and psychiatric social workers. 1. A CAPS member can discuss his or her par- ticular situation on the telephone (he need not give his name) with these professionals — NO CHARGE. 2. A member may have an initial full session (normally $60) at NO CHARGE. 3. If a problem - solving program is desired, a 20% discount is provided on services not already covered by insurance. Chiropractic Plan 1. Consultation — whether it be to determine the need of chiropractic services or the traditional consultation prior to care — NO CHARGE. 2. Complete examination on the first visit to the clinic that is most convenient to you — NO CHARGE. 3. All subsequent chiropractic care services, in- cluding x -ray, chiropractic treatment, therapy, and laboratory services at 20% discount. 4. The clinics will accept the assignment of in- surance benefits for covered members. Veterinary Plan 1. CAPS members may bring their pets to one of our participating veterinary hospitals for examination whenever necessary — NO CHARGE. 2. The fee for any treatment provided for the pet is 15% less than the hospital's usual and cus- tomary charges.
