Page 3 <br />This application does not include law enforcement data. <br />If your city wishes to participate in this application, your city's Responsible Authority must complete <br />and return to me the enclosed form entitled "Statement of Participation." <br />This must be done as soon as possible, as every effort will be made to submit the application on or <br />before January 2 so that the classifications will be effective on January 2 until rejected, modified or <br />approved by the Department of Administration.
