Page 2: NOTICE OF REQUIREMENT FOR TAX CLEARANCE <br />January 28, 1987 <br />You have the right to protest this action of the Department of Revenue by <br />requesting a hearing before the Office of Administrative Hearings - an <br />agency independent of the Department of Revenue. To request a hearing, <br />state the basis for your protest and submit along with the attached copy <br />of this letter in the enclosed return envelope. <br />The request for a hearing must be received within thirty (30) days of <br />the date of this Notice. A hearing will be scheduled by an Administrative <br />Law Judge and you will receive notification of the date and location of <br />the hearing. <br />If no action is taken on your part prior to the renewal date of you <br />license, the decision to deny you a tax clearance certificate will be <br />final. <br />Sincerely, <br />KAREN B.
