No. ISA No. 3 <br />Project Location: Little Canada Road, City of Little Canada, Ramsey County <br />Local Agency: Little Canada Local Project No.: 120156E <br />Contractor: FPI Paving Contractors, Inc. Contract No.: <br />Address/City/State/Zip: 3230 Rice Street, St. Paul, MN 55126 <br />Total Supplemental Agreement Amount $ 29,034.42 <br />This contract is between the City of Little Canada and the Contractor as follows: <br />WHEREAS: This current project includes street, sidewalk, and signal modifications <br />from Interstate 35E to Edgerton Street; and <br />WHEREAS: There was additional clearing, grubbing, and grading necessary on the <br />south side of the sidewalk between Schletty Drive and the cemetery to <br />provide a maintainable area adjacent to the sidewalk; and <br />WHEREAS: The City requested the final improvement of the rainwater gardens to be <br />modified for easier future maintenance which required some additional <br />grading and material removal; and <br />WHEREAS: A sprinkler system on the project was damaged and required repair; and <br />WHEREAS: The bituminous ramping around the raised iron in the street that was <br />required due to private utility relocation delays had to be milled prior to <br />final paving of the street; and <br />WHEREAS: Upon review of three existing storm sewer structures it was determined <br />that they should be rebuilt as part of the project prior to final street paving; <br />and <br />WHEREAS: Delays from the addition of signal work and the corresponding changes to <br />the pedestrian crossing requirements resulted in temporary ramp <br />installations and an increase in the overall time for traffic control; and <br />WHEREAS: The City Engineer and County Engineer determined that the work should <br />be completed with this project to avoid future impacts to the improvements <br />already being completed. <br />NOW, THEREFORE, IT IS HEREBY MUTUALLY AGREED AND UNDERSTOOD <br />THAT: <br />1. The Contractor agrees to complete the above described changes and additions <br />to the project. <br />2. This Agreement does not change any of the contract provisions unless otherwise <br />identified in this Agreement. <br />SAP 062-621-010 & 200-020-010 <br />
