ATTACHMENT A <br />(Resolution No. 16-71) <br />Conditions and Checklist for issuance of Special Event Permit to Chad Wagner d/b/a The <br />Tavern on Main, for music event on August 19 & 20, 2016: <br />(1) <br />The applicant has plans for parking vehicles including size and location of Lots, <br />points of access; and proof of permission for use of property upon which the special <br />event is to be held or upon which parking may occur, and in the case of County <br />right-of-way, proper documentation from the County approving the plans; <br />(2) For the concerts, the use of wristbands for limiting the number of persons is <br />indicated in the application and to keep underage drinking out. An entrance fee shall <br />be charged and entrance shall be by wrist bands only. The applicant shall also <br />indicate the maximum number of persons that shall be permitted, and the fire <br />marshal shall provide a maximum number of occupants based on the size of the tent; <br />(3) <br />Plans are submitted for fencing the location and the gates contained in such fence; <br />(4) Plans are submitted for providing toilet and lavatory facilities including the source, <br />number, and location, type and means of disposing of waste deposited; <br />(5) <br />Plans are submitted for holding, collection, and disposing of solid waste material; <br />(6) Plans are submitted to illuminate the location of the event including the source and <br />amount of power and the location of lights; based on plans provided, the fire <br />inspector shall determine if a state permit is required; <br />(7) <br />Applicant will provide security on premise as outlined in Special Event Application. <br />(8) Applicant will provide all staff training for employees that will be working this event <br />between August 1, 2016 and August 18, 2016. Training shall focus on prevention of <br />over service of alcohol and other means to safely serve customers. <br />(9) <br />Plans are submitted for sound control and sound amplification, if any, including <br />number, location, and power of amplifiers and speakers; all reasonably necessary <br />precautions shall be taken by the applicant to insure that the sound of the assembly <br />will not carry unreasonably beyond the enclosed boundaries of the location of the <br />assembly; <br />(10) Plans are submitted to meet the requirements of the State Fire Code, including but <br />not limited to the following: <br />i. Minimum of three 2A1 OBC fire extinguishers need to be on location in the <br />tent area; <br />ii. "No Smoking" signs need to be displayed throughout the area. <br />iii. No pyrotechnics are allowed. <br />iv. No parking within 20 feet of the tent. Measured from stakes. <br />v. No open flames within 20 feet of tent. Measured from stakes. <br />
