Schloer <br />Accountant <br />City of Lino Lakes <br />1189 Main Street <br />Lino Lakes, MN 55014 <br />Dear Ms. Schloer and members of Lino Lakes City Council: <br />Thank you for the opportunity to participate in the 1995-1996 <br />CDBG funding request process. Enclosed please find a funding <br />request on behalf of the Senior Outreach Program - in Lino Lakes, <br />administered by Anoka County Community Action Program, Inc. The <br />total amount being requested is $3637.00. <br />As you are aware, Pam Leibel serves your community as the Senior <br />Outreach Worker. She provides case management, referral services <br />and advocacy services to vulnerable or isolated residents of Lino <br />Lakes. She assists primarily low and moderate income seniors and <br />disabled adults in obtaining services such as food programs, <br />financial assistance, crisis intervention, energy assistance, <br />public health services, and others. Without her work in the <br />community, many of these individuals would not get the help they <br />need, left in their homes with inadequate food, medical care, or <br />other basic necessities. <br />The success of this program throughout the county is based on the <br />premise that assisting seniors and disabled adults to remain in <br />their homes results in fewer unnecessary nursing home or hospital <br />admissions, which lowers the financial costs for the entire <br />community. The program is a cost effective way of providing <br />crisis and long term solutions to those in need. <br />All support, past, present and future is very much appreciated. <br />It is evident that the city staff and elected officials are <br />committed to assisting vulnerable population with increasing <br />needs. <br />On behalf of seniors and disabled adults in the city of Lino <br />Lakes, thank you for your consideration of this funding request. <br />If you have any questions, please feel free to contact me at 783- <br />4741.
