In addition to a <br />relationship with District Memorial Hospital, Fairview also finalized a relationship with Chisago <br />Health Services earlier in 1994. Fairview's involvement in the health care community in Forest <br />Lake and Chisago is an exciting step forward towards the development of a fully integrated <br />regional health care system. <br />A key component to Fairview's development of a regional health care system will be the creation <br />of a new regionally focused medical center. The new medical center will consolidate the <br />operations of both Chisago Lakes Hospital in Chisago City and District Memorial Hospital in <br />Forest Lake. In the interim, approximately two to three years, Fairview will continue to operate <br />both Chisago Lakes Hospital and District Memorial Hospital as acute care facilities. <br />As a part of the lease agreement with the hospital district board in Forest Lake, Fairview will <br />establish an operational board of directors to oversee the operations of District Memorial <br />Hospital. The operational board will be made up of nine individuals including three community <br />members and a physician. <br />The purpose of my letter is to ask for your assistance in identifying potential candidates for the <br />community member positions. In your leadership role in the community, you come in contact <br />with a variety of people who may be the kind of individuals interested in community service. <br />Based upon your knowledge, we are asking for your personal recommendation of individuals that <br />would be potential candidates for the DMH operational board. <br />
