ACSO personnel - An incident report and First Report of <br />Injury paperwork must be filled out and turned in to the <br />transport coordinator, <br />b. Other Agency personnel - Copies of your incident report and <br />First Report of Injury must be forwarded to the transport <br />coordinator. <br />E. ES.0 ?E <br />1 Notify communlcations and give them the name, dob, description of <br />the escaped prisoner. Also include a direction of travel and vehicle <br />used if known. <br />2. Request that communications notify all local law enforcement <br />agencies of the escape. <br />3. Notify the detention Sergeant at the jail facility. <br />4. Complete an incident report on the escape and submit a copy to the <br />detention Sergeant before the end of your shift. <br />XXIII. PAPEHWOHKIDOQUMttjTATION <br />A. The transporting officers shall have in their possession the following <br />documents: <br />1. A filled out end signed detention form. <br />2. The ticket or tickets when applicable (Both white and hard copy) <br />3. The 104 form when applicable <br />4. Release paperwork when applicable (102 or 104 form) <br />5. A victim notification form when applicable <br />6. A signed end witnessed affidavit when applicable <br />7. Any special requests or Instructions <br />8. Any use of force, medical situations or unusual incidents must be <br />documented on the appropriate forms.
