➢ A third party will complete the background check. Upon receipt of the background check, <br /> management will transfer the information to an application checklist form. The checklist will <br /> score the applicant based on criminal, credit and landlord history. All household members 18 <br /> years of age and older must receive a passing score in order for the entire household to be <br /> considered for housing. <br /> ➢ If the entire household receives a passing score, they will move on to the Intake Interview. <br /> Households who do not receive a passing score will receive a Notice of Rejection. <br /> Intake Interview <br /> For households who score sufficient points to pass the application checklist, an interview with all <br /> applicant members of the household 18 years of age and older will be scheduled to obtain current <br /> information about the family's circumstances. This process is called an"intake interview". At <br /> the intake interview, management will complete the following: <br /> ❖ Conduct a private interview with all members of the household 18 years of age and older. <br /> ❖ Confirm all information provided on the eligibility application. <br /> ❖ Obtain family income and composition information needed to certify eligibility. <br /> ❖ Require all members of the household 18 years of age and older sign and date Minnesota <br /> Government Data Practices Act Disclosure Statement, 3rd parry verifications (verifying <br /> income and assets), HTC 24 (if applicable), HTC 15 (if applicable) and Tenant Demographic <br /> Profile (required first year of operation). <br /> ❖ In addition to the above, applicant's must provide a photo I.D. for all members of the <br /> household 18 years of age and older. <br /> REJECTION OF AN APPLICANT <br /> Applicants will be rejected if any of the following apply: <br /> ➢ Under Section 42 Regulations, most households where all members are full-time students are <br /> not eligible for housing. Exceptions are listed under student eligibility. <br /> ➢ All members of the household 18 years of age and older must score sufficient points to pass <br /> the application checklist. Failure to do so will result in the entire household being rejected <br /> for housing. <br /> ➢ The household must be income qualified per current income limits. <br /> Student Elidbility <br /> There are four exceptions to the limitation on households where all members are full-time <br /> students. Full-time student households that are income eligible and satisfy one or more of the <br /> following conditions can be considered to be eligible. Verification must be obtained to support <br /> the student status and applicable exception(s). <br /> Lakewood Apartments Page 2 10/13/2005 <br />
