Lino Lakes, MN 55014 464-5562 REGISTRATION PROCEDURE HOW TO SIGN UP Advance registration and payment is required for all classes. Be sure to notice registration deadlines mentioned after various programs. Please register as soon as possible,so we may adequately plan ahead. REGISTRATION/PAYMENT PROCEDURE Complete a registration form(located opposite page)in full for program attending. You may list up to two activities on an individual registration form,however a separate check is required for each program listed. Make your check(s)payable to the City of Lino Lakes, and be sure to include the correct registration fee. For your convenience, you may mail in your registration,walk-in to the Lino Lakes Parks and Recreation Office, call-in to the Parks and Recreation Office, or drop it off at one of the three locations listed below. �us MAIL-IN: to Lino Lakes Parks and Recreation Department, 7204 Lake Drive, Lino Lakes, MN 55014. DROP-OFF(24-HOUR): 1)The Lino Lakes Parks and Recreation Office, 7204 Lake Drive(Elm Street and Lake Drive) 2) Community Education Office at Centennial Elementary School 3)Lino Lakes City Hall (box located next to the front door) WALK-IN:to the Lino Lakes Parks and Recreation Department Office,7204 Lake Drive(Elm Street and Lake Drive). Open: Monday through Friday 8:00 am-4:30 pm. PHONE-IN:to 464-5562 during regular business hours, to make a tentative program reservation. Payment and registration form must be received by the Lino Lakes Parks and Recreation Office(7204 Lake Drive) within two working days after calling to hold the program reservation. If payment and registration are not received as stated above the tentative program position will be lost. CONFIRMATION Registrations will be processed on a first-come, first-serve basis. After issuing payment, assume your registration is confirmed and the activity will meet as scheduled. NO confirmations will be sent out. You will be notified by the Parks and Recreation Department ONLY if an activity is filled,activity is cancelled, 4NO010� or activity has been rescheduled. Please be sure to include daytime phone number on your registration form. The Parks and Recreation Department assumes no responsibility for reaching registrants who do not rovide a daytime phone number. lt�M4REFUNDS All fees will be returned if the activity is dropped due to insufficient enrollment or activity is filled before we receive your registration. 14
