At minimum, the following items <br />should be included. Please place a check mark () by those included. <br /> ___ A. Trash receptacles (indicate number) _____ <br /> <br />12. ADDITIONAL PERMITS <br />Additional permissions may be required including, but not limited to, the following: <br />A. Building Permit: Attach to this application a copy of your building permit(s) if you <br />are installing any electrical wiring on a temporary or permanent basis and/or if you <br />are building any temporary or permanent structures such as bleachers, scaffolding, <br />a grandstand, reviewing stands, stages or platforms. <br /> B. Fire Department Permit(s): Attach to this application any permission required by <br />fire regulations for such things as an open flame; fireworks or pyrotechnics; vehicle <br />fuel; cooking facilities; enclosures (and tables within those enclosures); tents, air <br />supported structures, canopies, or any fabric shelters. <br />C. Other Permits: Certain other licensing may be required by city, county and state <br />agencies, such as a large assembly license for gatherings over 1,000 people, <br />some types of food handling licensing, gambling, cabaret, etc. It is your <br />responsibility to check with the City Clerk or local authorities to determine what <br />licensing is required before final submission of this application.
