The Finance Department will provide each eligible employee with a written notice describing <br />rights granted under COBRA when the employee becomes eligible for COBRA coverage plan. <br />The notice contains important information about the employee's rights and obligations. <br />5.4 Deferred Compensation <br />The City will provide regular full-time employees the opportunity to participate in a pre-tax <br />retirement savings program through payroll deductions. This voluntary retirement saving plan is <br />designed to allow tax savings for employees. <br />Full-time employees will be permitted to receive a cash payment for 50 percent of their unused <br />annual sick leave accrual exceeding 720 hours at the conclusion of each calendar year. The balance <br />of the unused sick leave will be added to the employee's sick leave bank up to the maximum <br />accrual of 960 hours. The cash payment will be paid at the conclusion of each calendar year into <br />the employee's designated deferred compensation plan as authorized by the City. <br />5.5 Long -Term Disability <br />Long-term disability insurance is provided to regular full-time and part-time employees who work <br />at least 30 hours per week on a regular basis. Long-term disability benefits for part-time employees <br />are pro -rated based on the number of hours worked per week. Long-term disability benefits are <br />available based upon the terms and conditions contained in the contract with the long-term <br />disability insurer. Terms and benefits under the program are subject to change from time to time. <br />5.6 Safety Committee <br />The City of Lino Lakes recognizes its responsibility to provide the safest conditions for its <br />employees by ensuring all reasonable safeguards are established in the workplace. Employees are <br />responsible for observing all safety rules and for using all available safety equipment. Employees <br />are also responsible for reporting any unsafe condition to their supervisor. The efficiency of any <br />municipal operation can be measured directly by its ability to control unnecessary loss. To that <br />end, the Lino Lakes Safety Committee was established. The Safety Committee is made up of <br />employees who meet and examine the City's policies and procedures and review accident and <br />injury reports. The Safety Committee also reviews and updates the City of Lino Lakes Safety <br />Policies and Rules. <br />5.7 Employee Wellness <br />Employees may convert accrued sick leave or PTO leave to pay for their monthly health club <br />membership dues or online subscriptions up to a maximum of $60 per month, provided that the <br />employee maintains a sick leave balance or PTO balance of 80 hours. <br />Reimbursements are made on a monthly basis upon receipt of a completed form and evidence of <br />payment to the health club. Forms are available in the Finance Department. The reimbursement <br />is subject to federal, state and FICA tax withholdings. <br />17 <br />
