CITY OF LINO LAKES RESOLUTION NO. 13-136 RESOLUTION AMENDING THE CITY OF LINO LAKES FLEXIBLE SPENDING ACCOUNTS PLAN WHEREAS, the plan was adopted per Resolution No. 78-90 with the effective date of January 1, 1991, and WHEREAS, the plan was amended and restated per Resolution No. 95-156 with an effective date of January 1, 1996, and again per Resolution No. 00-119 with an effective date of January 1, 2001 and again per Resolution No. 03-07 with an effective date of January 1, 2003, and again per Resolution No. 05-181 with an effective date of January 1, 2006,and again per Resolution No. 07-204 with an effective date of January 1, 2008, and again per Resolution 11-13, with an effective date of December 31, 2010, and WHEREAS,the City of Lino Lakes wishes to amend the plan document effective January 1, 2014, to include the following; st •The Planchanges the Commencement of Participation from 1of st the month following six months of employment to 1of the month following 60 days of employment (change needed due to Health Care Reform Act). •The Plan incorporates the Heart Act Amendment. •The Plan incorporates the carryover of FSA funds (up to $500) from one plan year to the next. NOW THEREFORE BE IT RESOLVED, by the City Council of the City of Lino Lakes that: 1.The City of Lino Lakes Flexible Spending Accounts Plan be amended and adopted in the form presented to the City council effective as of January 1, 2014. Adopted by the Lino Lakes City Council this 9th day of December, 2013.
