12 <br />Plan Provisions <br />The following summary of plan provisions represents our understanding of <br />the substantive plan. <br />Medical Benefits <br />Eligibility The plan provides benefits to individuals who <br />terminate service with the City through service <br />retirement or disability retirement. <br />Service or A City employee who has left employment or is disabled <br />Disability and is receiving or eligible to receive a pension <br />Retirement under the Minnesota Public Employees Retirement <br />Eligibility System. <br />Other Employees who terminate employment before attaining <br />terminations service retirement eligibility are only eligible for <br />COBRA coverage. <br />Dependent Spouses and children (while dependent) of eligible <br />eligibility retiree are eligible for the plan while the eligible <br />employee is under age 65. <br />Survivor <br />eligibility <br />Upon the death of a covered retiree, coverage can <br />continue for a spouse until age 65. <br />Member Retirees or single eligible spouses are required to <br />contributions contribute for their coverage 100% of the "total <br />premium ".
