CITY OF LINO LAKES <br />RESOLUTION NO. 04-15 <br />RESOLUTION APPROVING CHANGE ORDER NO.1 AND PAYMENT REQUEST NO. 5 — <br />WEST SHADOW PONDS UTILITY AND STREET IMPROVEMENTS PROJECT. <br />WHEREAS, the construction of the project has been begun, <br />AND WHEREAS, design changes were required for the West Shadow Ponds Utility and Street <br />Improvements Project, <br />AND WHEREAS, a complete breakdown is spelled out in the Change Order; <br />NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LINO <br />LAKES, MINNESOTA: <br />Change Order No.1 resulting in an increase of $10,941.95 bringing the contract amount to <br />$338,751.00 and Pay Request No. 5 in the amount of $39,297.03 is approved for the West <br />Shadow Ponds Utility and Street Improvements Project. <br />Adopted by the Lino Lakes City Council this 26'h day of January 2004. <br />John J.
