• <br />CITY OF LINO LAKES <br />RESOLUTION NO. 04 -107 <br />RESOLUTION APPROVING CHANGE ORDER NO.2 AND PAYMENT REQUEST NO. 3 — 21ST <br />AVENUE STREET AND STORM SEWER IMPROVEMENTS PROJECT. <br />WHEREAS, the construction of the project has been begun, <br />AND WHEREAS, aggregate base, non - wearing course bituminous pavement, and 4" draintile <br />was added for the 21st Avenue Street and Storm Sewer Improvements Project, <br />AND WHEREAS, a complete breakdown is spelled out in the Change Order; <br />NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LINO <br />LAKES, MINNESOTA: <br />Change Order No.2 resulting in a increase of $13,646.98 bringing the contract amount to <br />$247,842.63 and Pay Request No. 3 in the amount of $28,708.73 is approved for the 21St <br />Avenue Street and Storm Sewer Improvements Project. <br />Adopted by the Lino Lakes City Council this 26th day of July 2004. <br />Ann J.
