ATTACHMENT A <br />(Resolution No. 08 -67) <br />Conditions and Checklist for issuance of Special Event Permit and Cabaret License to <br />Chad Miller d/b /a Miller's on Main, for music event on August 15 and 16, 2008: <br />(1) The applicant has plans for parking vehicles including size and location of <br />lots, points of access; and proof of permission for use of property upon <br />which the special event is to be held or upon which parking may occur, and <br />in the case of County right -of -way, proper documentation from the County <br />approving the plans; <br />(2) The use of wristbands for limiting the number of persons is indicated in the <br />application. An entrance fee shall be charged and entrance shall be by <br />wrist bands only. The applicant shall also indicate the maximum number of <br />persons which shall be permitted, and the fire marshal shall provide a <br />maximum number of occupants based on the size of the tent; <br />(3) <br />The plans for fencing the location and the gates contained in such fence; <br />(4) The plans for providing toilet and lavatory facilities including the source, <br />number, and location, type and means of disposing of waste deposited; <br />(5) <br />The plans for holding, collection, and disposing of solid waste material; <br />(6) The plans, if any, to illuminate the location of the event including the source <br />and amount of power and the location of lights; based on plans provided, <br />the fire inspector shall determine if a state permit is required; <br />(7) <br />(8) <br />(9) <br />The plans for security including the number of guards and the name of the <br />security company; in addition, the applicant is required to ensure that two <br />uniformed Lino Lakes police officers are present at the event during the <br />hours of 9:00 p.m. and 1:30 a.m. on August 15 and 16, 2008, or as long as the <br />Police Department deems necessary; <br />The plans for sound control and sound amplification, if any, including <br />number, location, and power of amplifiers and speakers; All reasonably <br />necessary precautions shall be taken by the applicant to insure that the <br />sound of the assembly will not carry unreasonably beyond the enclosed <br />boundaries of the location of the assembly; <br />The plans to meet the requirements of the State Fire Code, including but <br />not limited to the following: <br />i. Minimum of three 2A10BC fire extinguishers need to be on location <br />in the tent area; <br />ii. "No Smoking" signs need to be displayed throughout the area. <br />iii. No pryotechnics are allowed. <br />iv. No parking within 20 feet of the tent. Measured from stakes. <br />39 <br />
