The certificate of survey shall be prepared and signed by a <br />licensed land surveyor and shall include the following information: <br />(a) Scale (engineering only) at not larger than 1 inch equals 100 feet; <br />(b) North point indication; <br />(c) Existing parcel boundaries including dimensions and area; <br />(d) Existing legal description; <br />(e) Easements of record; <br />(f) Wetlands, delineated by qualified professional; <br />(g) Watercourses; <br />(h) Ordinary high water level (OHWL) of any lakes or Department of Natural <br />Resources (DNR) waters; <br />(i) Floodplain as shown on Federal Emergency Management Agency (FEMA) FIRM <br />map or as determined by amendments or revisions; <br />(j) All encroachments, including but not limited to pipelines and transmission lines; <br />(k) Existing grades with two -foot contours; <br />(1) Existing buildings, structures, and improvements within the parcel to be platted <br />and those 100 feet outside the boundaries of the subject parcel; <br />(m) Location, widths and names of all public streets, rights -of -way or railroad <br />rights -of -way showing type, width and condition of the improvements, if any, which pass <br />through and/or are within 100 feet; <br />(n) The outside boundary of the subject property, which shall be clearly marked with <br />survey monuments; <br />(o) Date of preparation and dates of revision. <br />(2) General location and site description. <br />draft revisions to Chapter 1001 subdivisions draft September 2011 Page 23 of 77 <br />
