11 January 1998 <br />Tom Decheine <br />8940 Pascal Avenue <br />Circle Pines, MN 55014 <br />Dear Tom: <br />We have received your letter in which you have requested a One Year Leave of Absence. <br />Your request has been evaluated based on a number of factors, including anticipated work load <br />requirements, reasons for the requested leave, and staffing considerations during the proposed <br />period of absence. Based upon those issues, Rick DeGardner, Public Services Director and <br />Linda Waite Smith, City Administrator will making a recommendation to the city council that <br />you be granted a six (6) month leave of absence. Your leave of absence will run from January <br />1999 to 1 July, 1999. In order to extend the leave of absence past 1 July, we will need a letter <br />from your physician stating the exact date of your return to work. That date will have to be some <br />time prior to 1 October, 1999. <br />During your unpaid leave of absence, you will remain an employee of the City of Lino <br />Lakes. You will not however, accrue seniority, sick or vacation time, or any related benefits. <br />We hope that your recuperation is going well.
