AGENDA ITEM NO. 3E <br />STAFF ORIGINATOR: Ry -Chel Gaustad, CMC City Clerk <br />DATE: December 9, 1999 <br />TOPIC: Transfer of Liquor License — Miller's on Main <br />VOTE REQUIRED: Simple Majority <br />BACKGROUND: <br />Attached is an application for liquor licenses, submitted by Mr. Chad Wagner. <br />Mr. Wagner plans to acquire Miller's on Main, (8001 Lake Drive) effective January 1, <br />2000. Sergeant Steve Mortenson conducted a background investigation and it is <br />available for inspection in the City Clerks office. Fire Chief Milo Bennett has confirmed <br />the building is up to code. The liquor fee has been prorated for six months and the <br />amount of $1662 has been paid in full. <br />• Liquor License $1500 <br />• Sunday Liquor License $ 100 <br />• Off Sale Liquor License $ 50 <br />• Cigarette License $ 12 <br />Staff is not in receipt of the certificate of "Dram Shop" insurance and proof of "Workers <br />Compensation." <br />OPTIONS: <br />1. To approve the application contingent upon receipt of a satisfactory certificate of <br />Dram Shop insurance and proof of "Workers Compensation. <br />2. To deny <br />RECOMMENDATION: <br />Staff recommendation is to approve the application contingent upon receipt of the <br />certificate of insurance, and proof of workers compensation. <br />
