TKDA
TOLTZ, KING, DUVALL, ANDERSON
AND ASSOCIATES, INCORPORATED
ENGINEERS • ARCHITECTS • PLANNERS
MEMORANDUM
To:
1500 PIPER JAFFRAY PLAZA
444 CEDAR STREET
SAINT PAUL, MINNESOTA 55101 -2140
PHONE:612/292-4400 FAX:612/292 -0063
David Ahrens Reference: Change Order No. 2
Copies To: 1997 Wearing Course and
From: John Powell P
Date: June 29, 1998
Trail Paving
Lino Lakes, Minnesota
Commission No. 11307 -02
The contractor completed work on the referenced project which was outside of the scope of the
original contract. The City negotiated the compensation prior to the contractor's completion of
the work. The work items are as follows:
1. As described in Change Order No. 1, the contractor disposed of material excavated from
Aqua Lane off of the project site. As there was no bid item for this work, the contractor is
being compensated on a time and material basis as detailed on the attached invoice. The total
additional cost is $1,522.50.
2. In order to place the bituminous wearing course on the roadway areas, the contractor must
adjust the manhole castings upward such that they match the new surface elevation. This
adjustment involves exposing the manhole casting, "rolling" down the rubberized manhole
seal, adding a concrete adjusting ring beneath the casting, replacing the casting, "rolling" up
the seal, and backfilling/compacting the disturbed area. Of the 67 manhole castings to be
adjusted; 36 required an additional section of rubberized seal, and 18 required additional
excavation to extend the seal onto the top of the manhole cone. As this involved more work
effort by the contractor than was expected in the contract, the contractor was compensated an
additional $200.00 for each of the 36 manholes, and an additional $300.00 for each of the 18
manholes. The total additional cost is 36($200.00) + 18($300.00) = $12,600.00.
Total Amount of Change Order No. 2:
$ 1,522.50
$ 12,600.00
$ 14,122.50
An Equal Opportunity Employer
