• <br />ATTACHMENT A <br />(Resolution No. 11 -72) <br />Conditions and Checklist for issuance of Special Event Permit to Chad Wagner <br />d /b /a Miller's on Main, for music event on August 19, 20 and 21, 2011: <br />(1) The applicant has plans for parking vehicles including size and location of <br />lots, points of access; and proof of permission for use of property upon <br />which the special event is to be held or upon which parking may occur, <br />and in the case of County right -of -way, proper documentation from the <br />County approving the plans; <br />(2) For the concerts, the use of wristbands for limiting the number of persons <br />is indicated in the application. An entrance fee shall be charged and <br />entrance shall be by wrist bands only. The applicant shall also indicate <br />the maximum number of persons that shall be permitted, and the fire <br />marshal shall provide a maximum number of occupants based on the size <br />of the tent; <br />(3) The plans for fencing the location and the gates contained in such fence; <br />(4) The plans for providing toilet and lavatory facilities including the source, <br />number, and location, type and means of disposing of waste deposited; <br />• (5) The plans for holding, collection, and disposing of solid waste material; <br />(6) The plans to illuminate the location of the event including the source and <br />amount of power and the location of lights; based on plans provided, the <br />fire inspector shall determine if a state permit is required; <br />The plans for security including the number of guards and the name of <br />the security company; in addition, the applicant is required to ensure that <br />two uniformed Lino Lakes police officers are present at the event during <br />the hours of 9:00 p.m. and 1:30 a.m. on August 19 and 20, 2011, or as <br />long as the Police Department deems necessary; <br />(7) <br />(8) The plans for sound control and sound amplification, if any, including <br />number, location, and power of amplifiers and speakers; all reasonably <br />necessary precautions shall be taken by the applicant to insure that the <br />sound of the assembly will not carry unreasonably beyond the enclosed <br />boundaries of the location of the assembly; <br />(9) <br />The plans to meet the requirements of the State Fire Code, including but <br />not limited to the following: <br />i. Minimum of three 2A10BC fire extinguishers need to be on <br />location in the tent area; <br />H. "No Smoking" signs need to be displayed throughout the area. <br />iii. No pryotechnics are allowed. <br />
