Fairview Clinic, page 8 <br />REAS, the proposed conditional use permit and development stage plan/ preliminary plat meet the <br />requirements of the City's zoning ordinance and subdivision ordinance subject to certain conditions of approval. <br />NOW, THEREFORE, BE IT RESOLVED that the City Council of Lino Lakes hereby approves the conditional <br />use permit for a planned unit development and the development stage plan/preliminary plat for the Fairview <br />Clinic expansion (The Village No. 2) subject to the following conditions: <br />1. Conformance with the requirements of the S.E.H. memorandum. <br />2. The landscape plan shall be revised to depict irrigation in all seeded areas; red maples in place of the <br />sugar maples proposed along Lake Drive; and two woodbine plants at the base of all three trellises <br />shown on the architectural elevation. <br />3. <br />Additional handicapped parking must be provided along the southwest side of the building just west <br />of the main entry of the clinic. <br />4. All sidewalks directly abutting parking stalls within the expansion area shall be increased to a <br />minimum of six feet in width. <br />5. At least one connection between the clinics internal sidewalk system and the city trail planned for <br />the Lake Drive frontage should be included. <br />6. Prior to the issuance of building permits for the site the applicant must submit a photometric lighting <br />plan for staff review and approval. <br />7. Prior to the issuance of building permits for the site the applicant must submit a revised site plan <br />depicting 24 foot wide drive aisles in the new construction portion of the parking lot and a <br />realignment of the drive aisle at the northwest corner of the building to staff's satisfaction. The <br />number of parking spaces provided shall at no time drop below the requirements of the Zoning <br />Code. <br />8. A development agreement between the city and the developer must be executed prior to the <br />execution of the final plat by the city. <br />9. The brick used for new construction must match that of the existing building, if staff feels the <br />difference is significant enough, the Lake Drive elevation must have an architectural feature or <br />embellishment to hide the transition from new brick to old brick as much as possible. <br />10. The applicant shall dedicate a 10 foot wide trail easement parallel to Lake Drive. <br />11. The proposed addition is subject to MnDOT and Anoka County Highway Department requirements <br />review, and approval. <br />• 12. Prior to the issuance of building permits for the site the applicant must submit a full revised set of <br />drawings depicting conformance with all of the above requirements. <br />
