ATTACHMENT A <br />• (Resolution No. 12 -65) <br />Conditions and Checklist for issuance of Special Event Permit to Chad Wagner d/b /a <br />Miller's on Main, for music event on August 17 & 18, 2012: <br />(1) The applicant has plans for parking vehicles including size and location of lots, <br />points of access; and proof of permission for use of property upon which the <br />special event is to be held or upon which parking may occur, and in the case of <br />County right -of -way, proper documentation from the County approving the <br />plans; <br />(2) For the concerts, the use of wristbands for limiting the number of persons is <br />indicated in the application. An entrance fee shall be charged and entrance shall <br />be by wrist bands only. The applicant shall also indicate the maximum number <br />of persons that shall be permitted, and the fire marshal shall provide a maximum <br />number of occupants based on the size of the tent; <br />(3) <br />Plans are submitted for fencing the location and the gates contained in such <br />fence; <br />(4) Plans are submitted for providing toilet and lavatory facilities including the <br />source, number, and location, type and means of disposing of waste deposited; <br />(5) Plans are submitted for holding, collection, and disposing of solid waste material; <br />(6) Plans are submitted to illuminate the location of the event including the source <br />and amount of power and the location of lights; based on plans provided, the fire <br />inspector shall determine if a state permit is required; <br />(7) <br />Plans are submitted for security including the number of guards and the name of <br />the security company; in addition, the applicant is required to ensure that two <br />uniformed Lino Lakes police officers are present at the event during the hours of <br />9:00 p.m. and 1:30 a.m. on August 17 and 18, 2012, or as long as the Police <br />Department deems necessary; <br />(8) Plans are submitted for sound control and sound amplification, if any, including <br />number, location, and power of amplifiers and speakers; all reasonably necessary <br />precautions shall be taken by the applicant to insure that the sound of the <br />assembly will not carry unreasonably beyond the enclosed boundaries of the <br />location of the assembly; <br />(9) <br />Plans are submitted to meet the requirements of the State Fire Code, including <br />but not limited to the following: <br />i. Minimum of three 2A1OBC fire extinguishers need to be on location in <br />the tent area; <br />ii. "No Smoking" signs need to be displayed throughout the area. <br />
