• • CITY OF LINO LAKES RESOLUTION NO. 13 -54 RESOLUTION APPROVING THE PLANNED UNIT DEVELOPMENT — FINAL PLAN/FINAL PLAT FOR THE VILLAGE NO. 5-LINO LAKES ASSISTED LIVING WHEREAS, the City has received an application for a planned unit development — final plan /final plat approval for property currently described to -wit: Outlot A, THE VILLAGE NO. 4, Anoka County, Minnesota; and WHEREAS, the City has approved a final plat entitled THE VILLAGE NO. 5 which will create the parcel on which the Lino Lakes Assisted Living addition is proposed to be constructed. This parcel will be described as: Lot 1, Block 1, THE VILLAGE NO. 5; and WHEREAS, the planned unit development — final plan/final plat approval request is based on the following submittals: 1. Memory Care Addition -Lino Lakes Lodging Plan Set dated April 23, 2013 and received by the City on April 24, 2013. 2. Village No. 5 Final Plat received April 24, 2013. ; and WHEREAS, at their meeting on April 10, 2013, the Planning & Zoning Board recommended approval of the planned unit development — final plan/final plat subject to certain conditions of approval; and WHEREAS, the proposed planned unit development — final plan/final plat meets the requirements of the City's zoning ordinance, subdivision ordinance, and the Lino Lakes Town Center Design and Development Guide subject to certain conditions of approval. NOW, THEREFORE, BE IT RESOLVED by The City Council of The City of Lino Lakes that the planned unit development — final plan/final plat for The Village No. 5 and the Lino Lakes Assisted Living Addition are hereby approved subject to the following conditions: 1. The developer shall submit an executed development agreement and all documents, fees and securities required therein prior to release of the signed Final Plat. 2. The developer shall record the plat within 90 days after the date of approval, otherwise the approval of the final plat shall be considered void, unless the developer requests and receives an extension from the City Council. 3. A Sign Permit application shall be submitted and approved by the City Staff prior to installation or construction of any signage.
