Rosemount, MN 55068 State Project No.: 0280 -55 Contract No. Z07502 Location: In Lino Lakes on CSAH 23 (Lake Dr.) From 1000 ft. south of Town Center Parkway to Apollo Dr. In accordance with the terms of this Contract, you are hereby authorized and instructed to perform the Work as altered by the following provisions. Due to a four week delay of the Award and Approval for S.P. 0280 -55, has pushed back the concrete wearing course date of placement according to the Contractors original schedule submittal beyond the Mn/DOT Standard Specification 2403.A placement date of October 1st. To remain on schedule as planned, the Project Engineer with the concurrence of the Mn/DOT Bridge Office will allow the Contractor to cast Bridge No. 02817 stage 1 deck to its full deck thickness of nine inches using a concrete mix design 3Y43 HFWC and eliminating the concrete wearing course in stage 1. This Change Order directs the Contractor to furnish all equipment, materials and labor necessary to construct the above stated full depth deck pour. All work will be performed in accordance with the pertinent portions of the Special Provisions, Mn/DOT Standard •Specifications, Mn/DOT Standard Plates and as directed by the Engineer.
