7. Submit a copy of the property owner's application for a grant, paid itemized invoice and <br />MDH Well Sealing Record to Anoka County Community Development to request <br />disbursement of grant funds to the City, who will pay the property owner. <br />ANOKA COUNTY <br />In its capacity as facilitator to the Wellhead Protection Group (Group), the County will: <br />1. Act as the fiscal agent to the Group accepting the MDH grant. <br />2. Maintain a current journal of grant fund balance and dedicated funds for <br />reimbursement of City approved cost -share grants. <br />3. Receive City notices that they intend to approve a well sealing cost -share grant <br />application. <br />4. Notify the applicant City that sufficient grant funds are available for each application, <br />holding that estimated amount for the City. <br />5. Notify the Cities when the grant funds have been exhausted. <br />6. Accept and retain copies of City records of grant applications, paid invoices and MDH <br />Well Sealing Records. <br />7. Distribute grant funds to Cities in accordance with State Auditor standards. <br />At the conclusion of the grant project (on or before April 30, 2016) the public water suppliers <br />(cities) will complete and submit the Grant Narrative Report to the Minnesota Department of <br />Health. <br />This Memorandum of Agreement is executed by: <br />Name <br />Representing <br />Date <br />Page 2 <br />
