S
sou me 91uy ry"sir l
Effectively Reporting an Incident
(How to Document)
One of the most important things you can do as a seller of alcohol is to document any
incidents that you may encounter. Proper documentation is an essential tool to use
when you are involved in an incident that may require you to recall vital information at
a later date. The following two scenarios are provided as examples of common inci-
dents (both on- and off -sale) and how these incidents should be documented.
Scenario 1 (On -Sale): A group of four individuals have been drinking at a tavern in a greater Minnesota town
with no cab services. One of the patrons is beginning to show obvious signs of intoxication. The bartender has
made the decision to discontinue service of alcohol. The patron's friends do not want to leave. The bartender
brings the intoxicated patron a soft drink. See below how to document this situation.
Scenario 2 (Off -Sale): A group of three young men come into a liquor store they each bring a different bever-
age up to the counter. The clerk asks for I.D. and only one of the 3 produce a valid I.D. The person with the
valid I.D. says, "I will pay for the others beverages." The clerk says that I cannot sell to any of you and asks
them to leave. See below to see how to properly document this situation.
On -Sale Scenario Incident Report
INCIDENT REPORTING RECORD
Dat. of Incident 2 , J© ,
Herne lin known)
Physical Das iptim: / /D • H
s..: Yin Ht.
