Council Member Dahl <br />moved its adoption: <br />introduced the following resolution and <br />CITY OF LINO LAKES <br />RESOLUTION NO. 00-119 <br />RESOLUTION AMENDING AND RESTATING THE CITY OF LINO LAKES FLEXIBLE HEALTH <br />CARE AND DEPENDENT DAY CARE PLAN EXPENSE REIMBURSEMENT PLAN <br />WHEREAS, the plan was adopted per Resolution No. 78-90 with the effective <br />date of January 1, 1991, and <br />WHEREAS, the plan was amended and restated per Resolution No. 95-156 with <br />an effective date of January 1, 1996, and <br />WHEREAS, the City of Lino Lakes wishes to amend and restate the plan <br />document effective January 1, 2001, to include the Final and <br />Proposed Cafeteria Plan Regulations issued March 23, 2000 <br />clarifying the administration of change in status elections under <br />cafeteria plans, and <br />WHEREAS, the City of Lino Lakes wishes to amend and restate the plan <br />document effective January 1, 2001, to include the newly proposed <br />Cobra Regulations issued January 1, 2000, and <br />WHEREAS, the City of Lino Lakes wishes to amend and restate the plan <br />document effective January 1, 2001, to increase the current <br />medical reimbursement coverage from a maximum of $2,000 to a <br />maximum of $3,000 per plan year. <br />NOW THEREFORE BE IT RESOLVED, by the City Council of the City of Lino <br />Lakes that the plan document be amended and restated January <br />1, 2001 to include the following changes: <br />1. The plan document will include the Final and Proposed <br />Cafeteria Plan Regulations issued March 23, 2000 clarifying <br />the administration of change in status elections under <br />cafeteria plans, and <br />2. The plan document will include the newly proposed COBRA <br />Regulations issued January 1, 2000, and <br />3. The plan document will increase the current $2000 medical <br />reimbursement maximum to $3,000 per plan year. <br />
